Licensing Authorities will accept applications for Gambling Act 05 premises licences from 21 May 2007. The forms to apply for a premises licence are below. These forms must be returned to the Licensing Authority relevant to your premises. The appropriate fee must also be submitted.
You will also find the forms to be used to give notice of your application to Responsible Authorities and to the public. You must give notice to the public outside the premises and in a local newspaper. Please consult the regulations for further details. Forms for ancillary applications in relation to premises licences (e.g. applications to vary or transfer) will be made available in due course.
NOTE: These forms cannot be amended in any way. If Licensing Authorities wish to add a logo to forms, they should access versions of the forms through the LACORS website.
